چکیده

A 41-year-old woman originally from West Africa presented with a 3-week history of generalized weakness. Clinical examination demonstrated quadriparesis, urinary incontinence, and reduced level of consciousness. The imaging of the neuraxis revealed multiple bilateral cerebral lesions, including basal ganglia and midbrain. Spinal cord imaging showed lesions in the conus medullaris (figure). The HIV test was reactive, with a CD4 count of 10 cells/mL (normal range 700–1,200/mL). CSF PCR forToxoplasma gondiiwas positive. CSF cryptococcal antigen, fungal and mycobacterial cultures, Epstein-Barr virus PCR, andMycobacterium tuberculosis PCR were negative. Conus medullaris involvement is an uncommon manifestation of toxoplasmosis in advanced HIV.

Cauda equina-conus medullaris syndrome as an isolated presenting symptom of intravascular large B-cell lymphoma: Case report and review of the literature

Intravascular large B-cell lymphoma (IVLBCL) is a rare non-Hodgkin lymphoma with variable clinical manifestations. Although neurological symptoms are common in patients with IVLBCL, isolated cauda equina-conus medullaris syndrome is rarely reported. We herein report a case of IVLBCL whose initial presentation was cauda equina-conus medullaris syndrome with neither dermatological nor hematologic...

Prevention of diagnostic errors in position of conus medullaris in adult patients

Background and aims: Finding the safe location of spinal cord for cerebrospinal fluid (CSF) during surgical procedures is very important due to its various nature for each patient as well as its potential peripheral nervous system hazards. The aim of this study was to find the relationship between the location of conus medullaris (CM) and gender, age and body mass index (BMI) in order to minimi...

Ventriculus terminalis of the conus medullaris: MR findings in children.

PURPOSE To determine the appearance of the ventriculus terminalis of the conus medullaris and the frequency with which it is seen on MR. METHODS Four hundred eighteen normal spine MR examinations were reviewed. RESULTS Eleven (2.6%) of 418 children, all younger than 5 years of age, demonstrated an ovoid, nonenhancing, smooth dilation of the central canal within the conus medullaris. Average...

A Patient with Splenic Marginal Zone Lymphoma Presenting with Spastic Paraplegia as the Initial Symptom

In this report, we describe the case of a patient with splenic marginal zone lymphoma (SMZL) who presented with spastic paraplegia as the initial symptom. A 42-year-old male developed progressive spastic paraplegia over 4 months. His neurologic examination revealed paraplegia with pyramidal syndrome, hypoesthesia below the T1 level, and anal hypotonia. Magnetic resonance imaging (MRI) of the sp...

Spinal MRI findings of guillain-barré syndrome.

Guillain-Barré syndrome is a relatively common, acute, and rapidly progressive, inflammatory demyelinating polyneuropathy. The diagnosis is usually established on the basis of symptoms and signs, aided by cerebrospinal fluid findings and electrophysiologic criteria. Previously, radiologic examinations have been used only to rule out other spinal abnormalities. We report a case of systemic lupus...
